OhMD Alternatives for athenahealth Practices (2026)
OhMD is one of the most widely adopted patient-texting products on the athenahealth Marketplace. If you are replacing it or outgrowing it, here is the honest field — texting platforms, full communication suites, and AI that works the phone as well.
The short answer
OhMD is a text-first patient communication platform with one of the larger athenaOne install bases on the marketplace — 228 practice connections and 64 reviews as of 5 September 2026. Practices look for an alternative for three reasons: they want the phone line covered as well as texting, they want the AI to complete work in athenaOne rather than route a thread to staff, or they want texting bundled into a platform they already pay for. The closest like-for-like texting alternatives are Klara, Artera, Spruce Health, Relatient and Solutionreach. If the real gap is that texting alone did not reduce phone volume, the alternative is an athenaOne-native AI platform such as Pretty Good AI.
The vendors, at a glance
| Vendor | Positioning | EHR focus | athenahealth Marketplace | Published pricing | Best fit |
|---|---|---|---|---|---|
| Klara | Text-first patient collaboration, now owned by ModMed. | Multi-EHR; parent company sells a competing EHR | Listed | No public list price. | Practices wanting the closest like-for-like replacement, with the ModMed ownership understood. |
| Artera | Patient communication platform across text, voice and email with an agentic layer and dedicated AI service squads. | Multi-EHR | Listed | No public pricing. | Enterprises standardizing all outbound patient communication on one system of record. |
| Spruce Health | HIPAA-grade communication platform combining a business phone system with texting, faxing and telemedicine. | Multi-EHR | Listed | No public list price on the marketplace listing. | Practices that want to replace the phone system and the texting tool at once. |
| Solutionreach (SR Health) | Long-established patient engagement and messaging platform. | Multi-EHR | Listed | No public list price. | Groups that value a long operating history over newer AI capability. |
| Relatient | Dash scheduling and omni-channel patient messaging for rules-driven, high-volume environments. | Multi-EHR | Listed | No public pricing. | Large groups whose scheduling rules are the hard part. |
| Pretty Good AI | athenaOne-only AI across voice, text, web and fax that completes the work in athenaOne rather than routing it to a queue. | athenaOne only | Listed | Pricing model published: platform per location, per-provider for modules switched on, voice metered per call. | Practices where texting did not reduce phone volume and the back-office work is the real cost. |
OhMD is not the problem most practices think it is
OhMD does what it says. It is well rated, widely adopted on athenaOne, and the teams that use it generally like it. Almost nobody searching for an alternative is doing so because the product is bad.
They are searching because of what happened after they rolled it out.
The pitch for patient texting is that it takes pressure off the phones. In practice it usually adds a channel without removing one. Patients who prefer to text start texting. Patients who call still call. The front desk now works two queues instead of one, and the phone volume that justified the purchase is roughly where it was.
That is not an OhMD failing. It is what a channel product does. A channel gives your staff a better place to do work. It does not do the work.
Three different alternatives for three different problems
If you want the same thing, better or cheaper: Klara, Artera, Spruce Health, Solutionreach, Relatient, Weave. Straight substitutions. Klara is the closest match at 221 athenaOne connections against OhMD’s 228, though its 4.1 marketplace rating against OhMD’s 4.9 is a real signal, and its owner ModMed sells an EHR that competes with athenahealth.
If you want the phone covered too: you are shopping for AI voice, not texting. Pretty Good AI, Assort Health, Talkie.ai, Hello Patient and healow Genie all live here. The question to ask each is what happens after the call.
If you want fewer vendors: the digital-front-door platforms bundle texting with intake, scheduling and payments. Yosi Health, NextPatient, DocResponse and Qure4u are the established options on athenaOne.
The consolidation math nobody puts on a pricing page
Look at what a typical multi-location athenaOne group is actually paying for: a texting tool, a reminders tool, a review and reputation tool, an intake and forms tool, a patient payments tool, and increasingly an AI voice tool on top. Six vendors, six contracts, six integrations, six places where a patient’s history stops.
That last one is the expensive part, and it never appears on an invoice. The AI answering the phone does not know the patient got a text yesterday. The review request does not know the visit was rescheduled twice. Staff carry the context between systems because nothing else does.
Pretty Good AI is built the other way around: one integration into athenaOne covering voice, text, web and fax, with the back-office modules — referral intake, schedule and capacity, revenue protection, KPI reporting — running on that same connection and the same memory of the patient. Fewer vendors is the visible benefit. One shared context is the one that changes how much staff time you get back.
An honest word on switching
Texting platforms are stickier than they look. Patients have your number in a thread, staff have workflows built around the inbox, and templates and campaigns took someone months to get right. Do not switch a working texting tool for a marginally better one.
Switch when the job you need doing is not the job the tool does.
Why practices pick us over Alternatives
Four parts, one platform, and each part uses the others. Most of the vendors on this site own one of these four well. The argument for us is that the same integration and the same patient memory carry all four, and that it is athenaOne only.
Patient engagement
We bring patients in and keep them close
We pick up the phone, day and night. Patients reach you by phone, text, chat or video. They book, reschedule, ask for a refill or get an answer. We call and text for you too — confirmations, recalls, and open slots to fill. Our AI remembers what a patient already told your practice, across every channel, so nobody repeats themselves.
Workflow automation
We do the staff work that piles up
A cancellation, a new referral, an overdue follow-up or a staff request starts a job. We do the steps, reach the patient if we need to, and hand the rest to your staff with everything they need. Your team sees the work in the Pretty Good AI Console or right inside athenaOne. Clinical decisions stay with your care team, always.
Referral management
We turn referrals into first visits
Referrals arrive by fax, portal and form. We track each one from intake to first visit, chase the missing information, reach the patient and book the visit. You see where every referral stands, and where the funnel leaks.
Revenue cycle
We keep revenue moving
Money leaks around every visit. We check insurance before the visit, track approvals so visits are not held up, call patients about balances, set up payment plans, and follow up on the claims your team hands us. Built to your billing team’s rules.
We only do athenaOne
One system means we go deep. We connect 500+ of the roughly 800 endpoints athenahealth exposes, and we read and write the real record — no middleware. Most vendors in this category connect ten or twelve. That depth is what lets a workflow finish instead of stopping at a handoff.
We build yours
If you can write down the rule, we can automate it. Your scheduling rules, your intake questions, your handoffs, your billing follow-up. And if another athenaOne marketplace app does a workflow you need, we can build it for you — same APIs, your rules, one vendor.
Live and measured
- 100,000+
- patient calls a month, at more than one customer
- About 60%
- of those calls handled start to finish by the AI at the largest deployments
- Hundreds
- of providers inside a single group
- 20
- specialties on our athenahealth Marketplace listing
The mix is deliberately wide: FQHCs, family practice and primary care, OB-GYN, behavioral health, orthopedics, pulmonology and sleep, gastroenterology, urology, urgent care and surgery centers among them. Different specialties break the front office in different places, and the rules that fix them are not the same rules.
Customer-reported results. Your numbers will vary by workflow, staffing, seasonality and call mix.
Month to month, no setup fees. We build your first workflow before you pay anything, then the first 30 days live are free from the day it goes live — not the day we start building.
We don’t sell AI. We build yours.
Frequently asked questions
- What are the best alternatives to OhMD?
- For like-for-like two-way patient texting on athenaOne: Klara, Artera, Spruce Health, Relatient Patient Communications, Solutionreach and Weave. For practices whose underlying problem is phone volume rather than texting: Pretty Good AI, Assort Health, Talkie.ai and Hello Patient. For groups that want texting inside a broader digital front door: Yosi Health, NextPatient, DocResponse and Qure4u.
- Why do practices switch away from OhMD?
- The most common reason is that texting did not reduce the phone volume it was bought to reduce. Patients who call still call, and staff now have a second queue to work. The second reason is scope: OhMD covers the message thread, so scheduling depth, referral intake, eligibility and revenue work stay with other vendors. The third is consolidation — groups running four or five point tools on athenaOne look to collapse them.
- Is Klara a good OhMD alternative?
- Klara is the most direct like-for-like alternative and has a comparable athenaOne footprint at 221 practice connections. Two facts are worth knowing before you sign. Klara was acquired by ModMed in February 2022, and ModMed sells its own EHR that competes with athenahealth. And Klara carries a 4.1 displayed rating on the marketplace against OhMD 4.9, the widest satisfaction gap between two comparable products in this category.
- What is the difference between a texting platform and an AI voice platform?
- A texting platform gives your staff a better channel to do the work in. An AI voice platform does the work. That distinction matters most on the tasks that generate the most inbound volume — booking, rescheduling, refill status, balance questions — because a text thread about a reschedule still needs a human to open the schedule, whereas an athenaOne-native agent can complete the reschedule in the chart.
- Can a practice run OhMD and an AI voice platform together?
- Yes, and many do during a transition. They are different channels and different failure modes. The question worth asking is whether you want two vendors and two integrations permanently, or one platform that covers voice, text and the athenaOne work behind both.
