The short answer

Affineon is an AI agent for the provider clinical inbox on athenahealth. It handles normal, insignificantly abnormal and CC-ed lab results around the clock, speeds up prescription renewals against protocols the practice sets, and publishes a rate of under $6 a day to triage the clinical inbox. Its listing shows 76 athenahealth Marketplace practice connections and a 5.0 average across 8 reviews as of 5 September 2026. Pretty Good AI is the AI operations platform built exclusively for athenaOne practices. We answer calls and secure two-way texts, bring patients in, automate staff work, turn referrals into completed first visits, and keep revenue moving, on production access to 820+ athenaOne APIs. The two products sit on opposite sides of the practice. Affineon works the queue the provider opens. We work everything arriving from outside the practice and the staff queues behind it. Lab-result triage is not a workflow we run today. If your phone, your referrals, your schedule and your balances are the problem, that is where we start, and it is a much larger surface.

Two different inboxes

Most comparison pages argue about features. This one starts somewhere simpler, because Affineon and Pretty Good AI are not fighting over the same queue.

Affineon works the inbox your providers open. Lab results arrive, most of them normal or insignificantly abnormal, and somebody with a license has to look at every one. Affineon handles the ones that meet the practice protocol, summarizes and trends the ones that need a clinician, and does the same kind of work on prescription renewals. It is a single-queue product with a published rate.

We work the other side of the building. The phone that rings 400 times a week. The referral that arrived by fax without the imaging. The patient who is overdue and was never called back. The eligibility check that failed on Monday and becomes a denial in six weeks. The balance nobody has touched since July.

Both are real costs. They are not the same cost, and they do not come out of the same budget line.

Say the honest thing first

Lab-result triage is not a workflow we run today, and we are not going to tell you it is.

Reviewing a result and deciding what it means is a clinical judgment. Clinical decisions stay with your care team, and that line is not marketing caution, it is how we build. Affineon works that queue with protocols the practice writes and a clinician in control.

So if the crisis in your practice is that your physicians are doing two hours of inbox after clinic every night, that is a queue we do not run today, and we are not going to claim otherwise.

Here is the part worth your attention: for most athenaOne practices, that is not the biggest number on the board.

The number most practices have not counted

Ask your front desk how many calls went unanswered last week. Most practices cannot say, which is itself the finding.

Then count the referrals sitting past day seven, the slots that went out empty yesterday, and the accounts receivable older than 90 days. Those are not staff frustration. Those are visits that did not happen and money that did not arrive, and they compound every week nobody works them.

That work has no specialty, no license requirement and no glamour, which is exactly why it stays undone in a short-staffed practice. Everyone triages what is in front of them. The queue nobody owns is the queue that quietly sets your revenue.

What we actually run

We are the AI operations platform for athenaOne practices, and the four parts run on one integration and one patient memory.

Patient engagement. We pick up the phone day and night, and we handle secure two-way text on the same integration. Patients book, reschedule, ask for a refill or get a question answered. We reach out too: confirmations, recalls, open slots to fill, surveys and review requests after the visit.

Task automation. Cases, orders, requests and the outreach attached to them. You choose the tasks and set the rules, we run the steps and track what is moving, waiting or stuck. When a provider releases a result, we run whatever patient outreach your rules call for, in your words.

Referral management. Referrals arrive by fax, portal and form. We track each one until the patient completes a first visit, chase what is missing, reach the patient, and book.

Revenue cycle. Insurance checked before the visit, prior-auth prep and status tracked so visits are not held up, balances worked, and claims followed up to your billing team rules.

Then the layer none of it works without. We combine your athenaOne data with the results of every call, message and task we handle, and use it to see where patients get stuck and where provider time goes unused. As conditions change we rebalance outreach and scheduling across providers and locations, within your rules. More provider time becomes care delivered.

Depth is the reason the work finishes

A lot of tools connect to athenaOne. Far fewer finish work inside it.

Creating an appointment or pushing a message reaches a handful of endpoints. Completing a stalled referral, resolving a failed eligibility check, prepping an authorization by payer or working a balance touches far more of the system, and every gap turns into a task for a human.

We hold production read and write access to 820+ athenaOne APIs, with no middleware in between, and our app runs inside athenaOne with no extra login. That is not a spec-sheet flex. It is the difference between a workflow that ends in the chart and a workflow that ends in somebody’s to-do list.

It is also why we only do athenaOne. Building to the full capability of one system and building to the features every system shares are different products, and we picked the first one.

What this looks like in a real practice

Picture a nine-provider primary care group, two sites, a front desk of five, and the schedule that goes with it.

The inbox problem is visible. Providers complain about it in meetings, and it shows up in every retention conversation. So it gets attention first.

The other problem is invisible, and it costs more. Roughly one in six calls never gets picked up in the busy hour, and those patients do not leave a voicemail, they call a competitor. Forty-odd referrals sit past a week because the sending office never sent the imaging and nobody has time to chase it. Six openings went unfilled yesterday because the waitlist lives in somebody’s head.

We start there. Calls get answered, every time, including at 6pm and on Saturday. Booking, rescheduling, refill requests and referral intake get handled and written into athenaOne. The waitlist backfills the slot that opened this morning. The referral at day nine gets chased, the patient gets reached, and the visit gets booked. Then we come back the following month with what changed and what to take next.

No new app for your staff. No workflow to relearn. You bring us the work that takes too much time or gets stuck, and it becomes ours.

How to decide in a week

Three questions.

Which pain has a dollar sign on it? Provider inbox hours are a retention cost. Unanswered calls, stalled referrals, empty slots and aged balances are a revenue cost, and you can put a number on that one this afternoon.

Who is accountable after go-live? A focused agent for one queue is easy to own. An operation spanning phone, referrals, scheduling and revenue is not, which is why we run it rather than hand it to you. When athenaOne changes or a rule changes, fixing it is our job.

Are you athenaOne? If you are, a platform built for only athenaOne will go further into it than one built to sit alongside many systems.

Count two things before your next leadership meeting: calls your team could not pick up last week, and referrals older than seven days. Those two numbers usually settle the order of operations, whatever else you buy this year.

Frequently asked questions

What is Affineon?
Affineon is an AI agent for the clinical inbox, integrated with athenahealth. It automatically handles normal, insignificantly abnormal and CC-ed lab results 24 hours a day, and surfaces the detail staff need to process prescription renewals faster, including whether a request meets the prescribing clinician protocol. Clinical documents such as discharge summaries are listed as coming soon. Its marketplace listing shows 76 athenahealth practice connections and a 5.0 average across 8 reviews as of 5 September 2026.
Is Pretty Good AI a replacement for Affineon?
Not today, and we will not pretend otherwise. Reviewing and releasing a lab result is a clinical judgment, and clinical decisions stay with your care team on either product. Affineon works that queue now. We start on the other side of the practice: the calls, texts, referrals, scheduling, eligibility, forms and balances that arrive from outside and the staff queues those create. A practice can run both without collision, because they touch different work.
We can only fund one this year. How do we choose?
Ask where the money is going. Inbox time is a burnout and retention cost, and it is real. Unanswered calls, referrals that never reach a first visit, unfilled slots and unworked balances are a revenue cost, and they are usually larger and easier to count. Pull two numbers this week: calls your team could not pick up, and referrals older than seven days. If those numbers are ugly, start on the outside of the practice, because that is where the lost revenue is.
How far does Pretty Good AI go into staff work?
If it is in athenaOne and a person, a piece of software or an AI could do it today, we want to go after it. Cases, orders, requests, referral documents, eligibility, prior-auth prep and status, forms, balances and the outreach attached to all of it. We start with your biggest pain point, prove it, and keep expanding month by month. Clinical decisions stay with your care team. Everything around them, from the first call through the visit and the bill, is in scope.
Does the under $6 a day figure mean anything for our budget?
It gives you a floor for one narrow job and nothing more. A published rate for clinical inbox triage does not price answering the phone at 6pm, chasing a referral that arrived without imaging, or working a balance that has sat for 90 days. Price each job against the money it is losing you, not against the cheapest number on any vendor page. We publish our own model: no setup fees, your first 30 days live are free, month to month after, and new workflows are priced when you turn them on.
Can a practice run both?
Yes. The overlap is small enough that it rarely needs negotiating. The one thing to settle up front is patient messaging: decide which system sends the message when a result is released, and write it down. Two systems both convinced they own that message is how a patient hears about the same lab twice.
