The short answer

MedXCom is an automated medical answering service from Giffen Solutions, operating since 2012 and on athenahealth since 2016. All five of its Marketplace reviews come from practices of one or two providers. Its listing shows 84 athenahealth Marketplace practice connections and 5 ratings, all five-star, as of 8 October 2026. The listing shows no reviews from larger or multi-site groups, and MedXCom publishes no call-volume figures. It forwards the practice line into a phone tree, captures the caller and reason for calling, and rings or texts the on-call provider. Its own published material does not describe AI. Pretty Good AI is the AI operations platform built exclusively for athenaOne practices, from referral and intake to billing, with production access to 820+ athenaOne APIs, 45 practice connections and 22 ratings all five-star as of October 2026. Both document after-hours calls in athenaOne; Pretty Good AI can also complete administrative requests on the call and write the result into athenaOne.

Two products that both end up in the chart

MedXCom and Pretty Good AI both take an after-hours call and leave a record of it in athenaOne, with different workflows for handling the request.

MedXCom captures the call. The patient says who they are and why they are calling, and the on-call provider gets rung or texted. The service has been operating since 2012 and on athenahealth since 2016.

We finish the call. The agent books the appointment, moves the appointment, answers the prep question, takes the refill request or the records request, and writes the result into the athenaOne record it belongs to. The on-call provider hears about the ones that actually need a clinician.

Start by counting what your nights are made of

Classifying after-hours calls helps show where administrative work can be completed on the call and where clinicians need to be involved.

Pull last month’s after-hours calls and sort them into three buckets. Routine scheduling: move it, cancel it, book it. Routine information: what time do you open, where do I park, do I need to fast. Clinical requests need a person with a license.

Each type of call can be handled differently: administrative requests can be completed during the call, while requests needing clinical judgment go to clinicians.

How MedXCom works

MedXCom routes callers through a customizable phone tree, can show athenaOne chart data to the provider during the call, and rings or texts the on-call provider. Its published materials describe operating since 2012 and supporting athenahealth since 2016.

Where the architectures diverge

MedXCom’s design assumes the call needs a provider. Everything in it is built to get that provider to the patient quickly and to document the exchange defensibly.

Our design assumes most calls do not need a provider, and sets out to prove it one call at a time. That is only possible with enough reach into athenaOne to finish the work rather than describe it. Reading the live scheduling templates, appointment types, provider groups and departments, checking the slot, writing the booking, creating the case.

That is the argument for production access to 820+ athenaOne APIs. A product that only has to capture a message needs very little of the system. A product that has to complete the request needs most of it. Most marketplace vendors pick a narrower wedge and build deep inside it; we carry significant depth across the whole of athenaOne.

The same reach is why the work does not stop at the phone. We run the full administrative call in 10 languages, English, Spanish, French, German, Hindi, Russian, Portuguese, Japanese, Italian and Dutch, with the same workflows in each.

The complication that decides it

The following is an illustrative workflow, not a customer story.

A multi-site primary care group runs an answering service for nights and weekends. Calls get to the on-call doctor, and the service records the encounters.

Then the group counts the calls. Just over 70 a week after hours. The on-call provider is being paged for roughly a dozen of them. The rest are a patient trying to move a Tuesday appointment, a parent asking whether the office is open on the Friday after Thanksgiving, and somebody asking what they can eat before a lab draw.

Those sixty-odd calls do not reach a provider, which is correct. They also do not get resolved. They wait for the morning, where they become sixty voicemails and callbacks for a front desk that already has a queue, and some of the patients have called a different practice by then.

Switch the routine two buckets to an agent that completes them and the night changes shape. The appointment moves at 9pm against the real scheduling template. The holiday hours question gets answered. The lab prep question gets answered from the practice’s own instructions. The dozen calls that need a clinician still escalate, by the same protocol, now carrying the name, date of birth and reason already written to the chart as a case.

A caller who starts describing something that needs clinical judgment goes to your clinicians. Clinical decisions stay with your care team.

What this costs to find out

Both vendors publish commercial terms, which is unusual in this category and makes the evaluation cheaper than most.

MedXCom describes flat rate pricing cancellable at any time with no term commitment, EMR integration at $19.95 per provider per month, a live operator at $1.15 per minute, and the first 14 days free. No setup fees. First 30 days live free. Month to month after. No annual lock-in. Pricing details are on the pricing page.

Use the published pricing alongside a one-month review of real nights. Count three things: how many calls finished without a human, how many escalated, and what happened to the ones that escalated.

The third number is the one vendors report least often, and it is the one that tells you whether after-hours coverage is working.

Frequently asked questions

What does MedXCom do?
MedXCom is an automated medical answering service from Giffen Solutions, operating since 2012 and on athenahealth since 2016. A practice forwards its line to MedXCom whenever it wants coverage. Callers move through a customizable phone tree, urgent callers confirm a callback number and state their name and reason for calling, and the system rings or texts the on-call provider. During the call it pulls chart data from athenaOne and displays it on the provider phone. An optional US-based live operator can screen according to practice instructions. [1] , [2] , [5]
How do MedXCom and Pretty Good AI handle after-hours calls?
MedXCom provides after-hours answering and documentation. Pretty Good AI handles the after-hours call and can complete administrative requests, as well as supporting the daytime phone, secure two-way text, referral and fax intake, schedule and capacity recovery and revenue protection on the same athenaOne integration. [1] , [5]
Who uses MedXCom?
All five of its Marketplace reviews come from practices of one or two providers: one solo pediatric practice and one two-provider multispecialty practice in 2017, one one-provider internal medicine practice in 2020, and two one-provider practices in 2025. Its Marketplace listing shows 84 practice connections and 5 ratings, all five-star, as of 8 October 2026. The listing shows no reviews from larger or multi-site groups, and MedXCom publishes no call-volume figures. [6] , [5]
What should an existing MedXCom practice measure?
Review abandoned calls during business hours, average hold time, bookings completed by staff, unworked referrals, and the depth of the order and tickler queue. These measures describe daytime work beyond after-hours coverage. [2]
How do we compare the two on a single after-hours call?
Bring one real scenario to each vendor and watch it end. A patient calls at 9pm wanting to move a Thursday appointment. Ask each vendor what happens next. With an answering service the request is captured and someone handles it later. Ask Pretty Good AI to move it on the call against your scheduling templates and show you the changed athenaOne record. Then ask how a new patient can be booked and what happens when a request needs clinical judgment. [2]
Does MedXCom use AI?
Its own published material does not describe AI. The site and listing describe a phone tree, voicemail capture, voice-to-text delivery of the patient message to the provider, real-time chart lookup and an optional human operator. [1] , [2]
What should a practice ask when evaluating after-hours workflows?
Ask each vendor to handle the same after-hours call live against an athenaOne environment. Which athenaOne records change when the call completes? What does the on-call provider receive? What share of calls needed a human? [1]
