If your selection criteria are vendor size and installed base, this comparison is over before it starts, and that is a legitimate way to buy. What follows is for the practices that want to know whether the products actually do the same thing.
They mostly do not
Phreesia’s center of gravity is the moment before the visit: intake, registration, forms, insurance capture, and collecting money. It is very good at that, at scale, across many systems. Voice AI and after-hours triage are additions to that platform.
That distinction is architectural, not a claim about quality: Phreesia began with intake and registration workflows, then added voice AI, while Pretty Good AI was built voice-first and extended into asynchronous channels.
Our center of gravity is the phone and what happens in athenaOne because of it. A patient calls, and the outcome is an appointment booked against the right appointment type and provider group, a refill in the right queue, a referral attached to the right order, a tickler closed. Then the same integration runs the work nobody has time for: waitlist backfill, no-show recovery, lapsed-patient reactivation, eligibility, prior-auth prep, denials and appeals, and per-call QA with KPI reporting.
That is why the athenaOne API count matters to us and does not appear on their listing. We connect 500+ athenaOne APIs out of the roughly 800 endpoints athenahealth exposes because completing work requires far more surface than capturing a form does.
Where the overlap is genuine
Appointment reminders, patient self-service scheduling, and some outbound campaigns. If those are the only things on your list, Phreesia does them and you already have a relationship with them.
Where we would push back
If a practice tells us they are buying Phreesia to solve phone volume, we would ask what happens to a call that needs a decision — a reschedule with a provider constraint, a refill for a controlled substance, a referral with a missing order. Intake platforms are not designed to resolve those, and the calls that overwhelm a front desk are exactly those.
